Utilizing the BHCS nursing professional practice model, the registered nurse (RN) is a licensed professional who coordinates patient care delivered by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops, implements the plan of care and evaluates the patient’s response. During this process, the RN communicates patient data to the health care team, makes recommendations for patient interventions, collaborates with other health care providers to deliver care and documents outcomes. The RN is responsible for creating a plan of care that is sensitive to diversity, age appropriate, individualized to the needs of each patient and promotes safe passage. RN promote safe passage for their patients by using knowledge of patient needs and the healthcare environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient’s condition and the competencies of the employee. The RN uses clinical judgment, clinical inquiry, advocacy, caring practices, collaboration and systems thinking in the provision of nursing care. The RN facilitates the learning for patient, families, staff and community through effective teaching.
